In what world can you do this? I used to work part-time and it just kept creeping up until I was working 45-50 hours anyway but not getting the full benefits offered.

There is really no cure for this. I know it's frustrating, but even the best and most empathic don't get the mental load thing and how exhausting it is.

Agree with the plan to just stop doing so much. Do 50% of the laundry and leave the rest, especially his laundry. Make the doctor's appointment for the kids but have him take them. Don't make dinner for several days per week. The kids will be fine with cereal and sandwiches, which he can prepare. Give him a list at the start of the week of the things he needs to do and then, after a few weeks, tell him he needs to take over organizing. He has to FEEL the burden of what you do before he will ever change his behavior and or mind.
