If I had to guess, I'd say that small town attorney is killing it, and that's why they can afford for the wife to SAH *and* have a full time nanny.
I have relatives in certain small towns. They do very well and live very well. One is a real estate broker. Doesn't sell houses but land. Significant acreages. Also invests. These are not fancy small towns but there's more niches for doing well than people might imagine who aren't familiar with small towns.
