It isn't necessarily "bad". I think one important part is to find out what "moderate support" is being given.

Moderate support = needs the teacher’s help to figure it out

Subject: How common are 2s on ES report cards?
My DD got a few 2s and mostly 3s in third grade in the first quarter. Her 2s were in math and citizenship. I knew that was BS since she had been doing multiplication and division as a first grader and consistently brought home work that was entirely correct, and she had never been a disruptive kid in class.
When asked, her teacher said that while she was doing the problems correctly, and understood the concepts, she wasn't doing things "her" way. And that they generally don't give out 4s in the first quarter, because kids aren't going to show mastery at that early stage. And the kids graded themselves on the citizenship questions.
DD is a 9th grader now, and that teacher has been her worst teacher to date (there were issues beyond what I outlined). Somehow, she magically ended up with 4s in everything by the end of the year, and 4s consistently every year since in ES. She has also not brought home anything less than an A since middle school, in all honors/AP, and is accelerated in math.
IOW, talk to the teacher. Maybe there is a problem, and maybe there isn't. No way to tell by just looking at the report card.
