Are the creatures doing this for a reason or is it just a side effect of their nature. At some points in the film it appears that the creatures are chasing humans, mimicking voices, etc. If they had some physical presence and just wanted humans to die why wouldn't they attack the derfenseless humans. Why would they want them to die in the first place. The creatures could be like the cuttle fish whose rapid color changes cause some kind of neurological damage even though they may not necessarily be trying to harm humans.

I noticed that the Car Gang and the River Guy also had normal eyes. The “things” aren’t always around.

This was easily theater worthy.
Were there unrealistic parts? A boatload, but I chose to turn a blind eye (pun) to them.
I really liked the actors and thought "girl" was particularly sweet. I liked Sandra Bullock and Tom's chemistry together a lot.
Side note, am I the only one who thinks that Tom resembles a much more attractive version of 50 cent?

How did Tom kill the bad guy after getting the crazy eyes? I thought because that was his worst fear, that someone would hurt Mallory and the kids.

I was wondering if the guy showed up because of the babies. Strange they both went into labor at the same time. Did he do something? I also thought he was there to see if one or both were psychotic like him, especially with the mom “deciding” who would look on the boat. I thought for sure that Girl was psychotic and could look.
I kind of thought they should have let the kids look. The creatures show you your worst fear, but they are blank slates, no fear. No comprehension. Perhaps they could grow up evolved to process them.
