so basically she has no sense of self-worth outside of her looks and its deeply ingrained, given her family and social circle. Growing old is not going to be easy for either of you if she doesn't develop esteem through other things. Instead of telling her she looks 'great' as she is now, how about helping her develop other skills and ways of seeing herself as worthy, accomplished, beautiful that have nothing to do with looks?
Meh, what has she accomplished? Sounds like she made herself into plastic sex doll, OP locked it down young, and she popped out a couple of brats. Nothing to put on a resume.

Tell her I have a very critical mother who lives in Europe.
Tell her the first words my mother has said, when she comes to pick me up at the airport, instead of "hello", have been "wow your belly's big".
Tell her that despite my history of anxiety and panic attacks and pointed maternal comments...
I STILL GO
And am mature enough to have a lovely time.

Yup, that was me, the guy with the 38 y/o tall wife and 2 kids. Sorry for creating confusion, but I'm going through the same thing as the OP, only my wife has never cancelled a trip due to it.

I have a Colombian friend - he is a post doc making so much money - and he is paying for his sister to fy back to Colombia to have liposuction because it is apparently of paramount importance to look like a sex doll. Not healthy...

This makes more sense if the anxiety is tied to seeing her family in Mexico. There are probably deeper issues at play there than just her body. Or, she might have a family that does a lot of body shaming. This makes more sense now.
