They probably assume travelers use the "do not disturb" sign AND also lock the extra interior lock (as most travelers do). I'm sure that the hotel woman was as surprised as OP. She will probably file a complaint herself.

Weird that they assume that in a place that does not have interior locks. Strange.

If someone is in a room without an interior lock (if that even exists, never seen it and travel frequently), you would expect some to be especially likely to use the do not disturb sign.
