Not always. My family from the South uses "Little X" all the time and it's not a dig, just a statement. Lots of their kids are named after their fathers and so become "Little Whatever." John's son is "Little John." It's affectionate, not a dig.

It sounds like she's making amusing digs at her outrageously tall kid, not your "short" 1.5yo kid.

As I understand it, she send your son clothes -- which I would be thrilled about (albeit they may not be your taste) and a note directly to him, which says something like:
Hi Little Opie, I hope these clothes fit by the time you grow into them and they are not out of style! Love Aunt Marge.
