It's not crazy if you understand how it came to be. It was started solely by alums who wanted to give these very generous scholarships in order to lure to UVA students that otherwise might go Ivy. My DS got his school nomination and made it to regionals but not to the finals, which are actually done at UVA. A friend of his (same church) did get it, though. Yes. Jefferson Scholars are selected by a completely separate process - outside of UVa. Admissions is part of UVa. It is uncommon but it has happened in the past.

I know several nominees from various schools that didn’t get UVA offers - but I’ve never heard of anyone being offered a Jeff but not getting accepted into UVA.
this is correct: when the Jefferson gets to the final round, which is february, they communicate with UVA and they all get in. Before that, even if a semifinalist, it is not guaranteed.
This must be somewhat new, as there definitely have been a small number of cases in the past where a student was offered a Jefferson by the Alumni Association, but UVa did not offer same student admission. This sounds like a useful fix to how it works.
