How does he do at school? Any concerns from teachers? Or is it just sports?

If just sports, try some sort of martial arts or as pps said swimming. Opt out of team sports for a year or two until he’s more confident. You could also try track/cross country. Just keep him active. But sports may not be his thing. And that’s ok.

Another vote for swimming.

I made all my kids do swim lessons then swim team. It’s a safety issue.

Once they are good enough I know they can crank out many laps in a row in decent form, then it’s optional.

Get him on those meds

I put my son in swim lessons at a young age so he wouldn't drown and took him to the playground every day, sometimes twice a day. He didn't like team sports for many years. Something clicked in 7th grade and he started liking team sports. Now at 9th he's trying out for the high school basketball team. He was always active and athletic but didn't have the attention span or maturity for team sports. My main goal was getting his energy out and keeping him healthy. Mission accomplished.
