You can fill out the one form to request screening. You don't have to do the questionnaire or the work samples. You can make it as much or as little work as you feel is needed.
It is added work and that is one reason the Auditors suggested removing parent referrals as an option. There is a massive discrepancy in who chooses to refer and who does not. part of that is due to lack of knowledge of the program and the referral process requiring some extra work that not every parent is capable of completing. As such, the referral program is over used by higher income families and barely used by lower income families.

Parents go nuts over these scores. They want to avoid too many questions and complaints before the AAP packets are turned in.
