She has been visibly pregnant for the last two months, and she already knows the gender of the baby, so not newly pregnant. He surely knew she was pregnant back when he took photo.

The pic is intentionally unflattering and photog was an a-hole but I doubt he knew she was pregnant.

Why should you care or not depends on your own values, believes and upbringing. Anyone who support this photographer hates women. There is no higher standard in respecting a woman for a fact that she is a woman, especially when she is pregnant. This is a basic common sense.
