I doubt she was pregnant when the photo was taken. And if she was, he didn’t know. Furthermore, if she was pregnant at the time of the photo, she shouldn’t be getting crap injected into her body.

She has been visibly pregnant for the last two months, and she already knows the gender of the baby, so not newly pregnant. He surely knew she was pregnant back when he took photo.
