The filter seems to be completely gone. (Dementia, especially frontotemporal dementia, can damage the frontal lobes of the brain, which act as the brain's "filter." This damage can impair the ability to inhibit inappropriate thoughts and behaviors.). If he were normal, he'd know it's not socially acceptable to say what he said, even if he thinks it inside his head.
