She didn't correct to Countess, she wanted the driver to call her Mrs. Bethenny is so informal, she wants the world up in her business, she doesn't get any level of boundaries.

I'm basing my view on her self worth on more than just this recent incident. My opinion of her goes way back to when she corrected Bethenny for the way she introduced her to the chauffeur. It's not Luann, it's Countess de De Lessups. Her persona as the Countess was her whole thing in those early years and she way overdid that class with the countess/pseudo-grandiosity stuff to overcompensate for her feelings of inadequacy.
Luann alone is not enough. She needs a title, a man or some allure of grandeur to validate her worthiness. Her self worth is wrapped in what other people think of her. Above all, she is concerned with how it looks and what others think, keeping up appearances at all costs.
