This actually happened to someone I know where in college the school never sent the transcript and they got rejected from a study abroad program and once they figured it out it was too late so they had to find another program.

+1 I can't believe it took 3 pages to get here. If UNC has the first transcript, they come back and ask for the last transcript. OP's DD isn't even involved at this point. It's all between the HS and the college.
The only explanation is the counselor is lying to cover his ass.
This is not the way it worked for my child. He had to request a final transcript be sent to the school he was attending. I assume the process is pretty automated. Student selects the school which has a code. HS electronically sends the form as requested. There is no one putting the transcript in an envelope and noticing that an earlier one went to a different campus.

OP, it's quite possible your DD selected the wrong item on the drop down menu, but it would be nice if the system gave her some sort of confirmation of school selected and send date. I hate it when there is an important item to do and the software or web interface gives you no idea whether your request went through or whether it was acted upon.

Maybe she accepted a waitlist later. Maybe she got a scholarship later. She’s an adult and it was her fault.
You don't know if it's her fault. You only know the school aid it was her fault, not theirs. And only they have access to the ways to verify that claim. You do know that people lie, right?

I’m all for SLACs - I went to one and I expect there’s a strong chance my kid will go to one - but that’s a dumb statement. It might be that the college would contact the HS but I think it’s more likely they’d contact the student. It’s the student’s responsibility. HS need permission to release info to a college and if the student hadn’t properly provided it (which sounds like may well have been the case), it would bounce back to the student anyway, so might as well go directly to the source.
And either way, UNC *did* have a system in place that worked. They held (not rescinded) the placement and contacted the student. The problem was resolved (it sounds like relatively quickly) and a lesson was learned about double-checking submissions and maybe even following up to confirm successful completion. Done.
IDK my friends son went to a SLAC, his dad died the 1st month there. He went home, went to funeral, went back to school and never left his room for 2 months. The SLAC never noticed, didn't check on him, he literally had food delivered. His roommate finally called my friend, the mom, AFTER 2 MONTHS.
OTOH, her other son who was a sophomore was at MD state college, his professors gave him an alternate schedule for his projects to be turned in, a counselor visited him a few times, the RA check on him weekly, he received information about a support group.
