From what I hear--a lot from doctors-- it is a combo of private equity, vertical integration (healthcare/pharmacy/insurance like United Health Group)--ACA didn't allow doctors to own hospitals, it turned out doctor greed is nothing compared to PE and insurance greed. DOJ (under Biden) has an antitrust probe into UHG right now.
