Which state?
Some are issued at the county health office. Some come from the state's health office.

If you live in DC here is the website to order a new birth certificate. It looks a bit sketchy but is the official site.

You can also go in person to the Department of Health and save the $16 processing fee (still need to pay $23 for the actual certificate).

Some of might send our passport app in plenty of time yet still experience a glitch, like the poster's friend who had to be called with questions.
