Another way to think about is that, even on his lucid days, Trump uses language purely in service of his goals, and Truth is not one of his goals. He is a dealmaker / negotiator of the "car salesman" variety and he'll say anything to pressure people to go his way, and craft fictional rationalizations to put opponents on the back foot. He doesn't operate at the level of intellectual debate, he operates at the level of snarling and baring teeth and foaming mouth to scare people into submission.

You're giving his supporters the benefit of the doubt, and you shouldn't. MAGAats are happy when they see ridiculous statements from Trump - to them the entire "normal" international system is corrupt, and needs to be torn down. In fact, for them almost every institution deserves to be torn down except for the military, border patrol, and their own Church/neighborhood/family.
