Another way to think about is that, even on his lucid days, Trump uses language purely in service of his goals, and Truth is not one of his goals. He is a dealmaker / negotiator of the "car salesman" variety and he'll say anything to pressure people to go his way, and craft fictional rationalizations to put opponents on the back foot. He doesn't operate at the level of intellectual debate, he operates at the level of snarling and baring teeth and foaming mouth to scare people into submission.

This has to be the reason why MAGA is so oblivious to the fact that he is not well. I see reposts of his crazy SM posts every day on my SM feed, just as I'm assuming many on this board do, but I don't think his followers do. I think most "libs" know more about the crazy stuff he says and does than his most ardent followers do.
You're giving his supporters the benefit of the doubt, and you shouldn't. MAGAats are happy when they see ridiculous statements from Trump - to them the entire "normal" international system is corrupt, and needs to be torn down. In fact, for them almost every institution deserves to be torn down except for the military, border patrol, and their own Church/neighborhood/family. It's an extreme form of nihilism, as they've stopped believing in the very concept of institutions larger than the tribal unit.
