It's a common look amongst the male elders in my culture. A no drinking culture but tons of meat, fried food, sugary foods, sodas, sweet teas with very little to no emphasis on working out. Fortunately a lot of the next gen males have figured out that this is genetic so they focus on eating better, cardio, strength training.

My grandfather had that. And a massive heart attack. Unfortunately it’s a sign of heart issues.

OP - I would strongly encourage you to get him to change. We only have one life and it's not long.

Living with those issues and lack of exercise will likely reduce life span. Maybe only live to 60 or 65 when you good have lived to 80.
