Lehigh has a LEAP program and allows you to cross major between departments. They have fabulous engineering program and recently added a college of health.

I would consider something like a chemical engineering major for undergrad and a master's in international health policy. I'd think any college with strong engineering would be a good fit.

I shared all of this with my kid. He is grateful for all the suggestions and will research.

I like the way he thinks! He did agree that graduating debt free needs to be a priority, but for us that means finding a school that meets full need, or offers great merit.

He can gain the engineering experience he needs to take with him for water system design in developing countries.
He may also want to consider the Corps of Cadets at VA Tech. The Corps students get free tuition and then can choose to serve in military to as civil servants in the Corps of Engineers. I feel like many of the posters on this thread so far know very little about civil and water systems engineering, or engineering in general...
