We both work full time and when the third was born, the older two were 8 and 3. The oldest was in public school and got himself to school and back. The middle one was in full day preschool , we drove. The baby was with a live in nanny for three years. By that time the middle one was in public school as well. So we have only one pickup and drop off. The older two now have activities on the same weeknight and I drive them to and from while DH takes care of the youngest.

We have an older child & younger twins (I don't want to hear "why do you choose to have three kids and complain about") It's difficult. We both work full time. Oldest now in full time school and extended hours and youngest still home w/nanny. it's doable.

Assuming there isn’t a giant age spread, the older kids are still young. Kids under 6 don’t have extracurriculars, at least not unless Mom is really pushing something in particular. But they are totally unnecessary, especially several days per week.

Hiring a (second) nanny so you don’t have to bothered with school pickup is a money throw away. But to each their own
