Terribly and I can see why his family his making this public now. He was prolific. They wouldn't want him to diminish his portfolio by struggling in roles or just dropping out.
Bruce Willis — who starred in no less than eight movies in 2021 — has been given his own category at the 42nd annual Razzie Awards. “Worst Performance by Bruce Willis in a 2021 Movie” is the name of the new category, and the nominations include every single film he starred in last year, including American Siege, Apex, Cosmic Sin, Deadlock, Fortress, Midnight in the Switchgrass, Out of Death, and Survive the Game. Midnight in the Switchgrass was dead last in Vulture’s ranking of straight-to-VOD Bruce Willis films, with our reviewer commenting, “Bruce Willis is a nonessential worker here.”

Honestly I am happy we have an explanation for why his acting was so terrible in recent movies, it felt like he didn’t care but instead was just struggling.
So that begs the question why he did all of those movies last year.
Not clear if he was contracted to do the movies before his symptoms / diagnosis, or if symptoms began and he couldn’t get out of the projects, or if he was symptomatic and still wanted to continue filming for some reason.
Seems unkind to release details about his poor acting now knowing this diagnosis.
I think it's more likely the patient does not measure the impact of his disability, and that the family has stepped in to persuade him not to engage so much. Part of that is to go public. No knowledge of this particular case, but this is usually what happens.
This. Happened in our family. Person goes into denial and wants the normalcy of work.
And when that happens, it's really hard to tell the person they CAN'T work any more. Because when is the line where they can vs can't make their own decisions. Especially if they aren't harming anyone. And aren't even "that old" yet.
If I were his current wife, though, I wouldn't let him go live with his former wife. I'd want the time left with him. But she's got little kids to handle on her own now. So, no judgement.
All the more reason for older men in Hollywood and elsewhere to quit starting second families in their late 50s, 60s, 70s. We're all mortal. Deal with it and quit making poor decisions just to feel like you're a young man.
If it's more serious than they are letting on, he may not even remember who his current wife is and only remembers Demi at this point. It happened to my dad; he could only remember things that happened 20 years before.

+1 happens to many families with older parents
This makes me especially sad. My dad had me at 55 and died at 63 having been sick for the last year or so of his life. I have very few memories of him in general, most involve him being in the hospital or sick/weak/near death so they are memories made from trauma, not normal memories. I think I can count on one hand “other” memories I have of him and all are just momentary and insignificant.

Take this for what it is - Twitter being Twitter - but there's some people speculating that his "handlers" (managers, agents, whoever) were pushing him to work as much as possible while they could still wring money out of him. That he was essentially in an abusive, predatory situation.
I really hope that's not the case. I've loved him since Moonlighting, and this news is so sad. I hope he is feeling ok.
I feel like this is potentially true but also likely that his family was attempting to convince him to step back and he didn't want to. His ex wife is Demi Moore and he is an incredibly successful movie star. Demi has not worked hard in a long time. She is the kind of actress now that does something if she thinks it sounds interesting or if someone asked her to guest star. But she clearly isn't working to support herself. And their family is blended and incredibly close knit, so I just don't see any of them pushing him for a payday. And they would actively be fighting 'handlers' if they felt that was happening.
