This is false. A quick google search will debunk that, but I think this may be a cultural choice, not all cultures value big goofy smiles in wedding photos, especially for women, they are supposed to look serious as it is a serious occasion.
I did a google search. I scrolled through more than a hundred photos and she's not making an obvious smile in any of them.

Her eyes aren't smiling either, maybe that's a residual effect too? I don't know I genuinely hope she is truly happy.

She's certainly had reconstructive surgery at some point, but who can blame her? Even if she had a nose job (I don't think she did, looks like makeup), it's completely different than some rando from Potomac getting a nosejob.
She looks a bit thinner in the face. May just be a function of age/maturity or maybe she's lost weight.
