It's bloat. He's retaining fluid. That's what happens in late stage heart disease. I saw my aunt four days before she died of heart disease. Her ankles were huge, and she could barely walk (and she was a tiny woman). She was coherent, if a bit forgetful, and we had a lovely conversation. Four days later, she was gone.

Dementia damages parts of the brain that control how we move and judge distances. This makes a multi-step task like putting a medal on someone very hard because the brain cannot send the right signals to the hands. A major cause of this is apraxia, which is a brain condition where a person loses the ability to do normal, everyday physical movements, even though their muscles are still strong. Because of apraxia and poor depth perception, a person with dementia will fumble with the ribbon, struggle to aim for the head, and drop the medal.

The question is when is he finally going to implode? Or will his handlers manage it so well we will never know until they can’t hide the fact that he’s in a coma or dead. There are people very, very intent on not necessarily protecting him but the corrupt system they trusted him to establish. He’ll eventually die, but his owners are not going to let go so easily.
