His doctors state that kidney and heart failure have been ruled out, and that swelling in his hands is due to his chronic venous insufficiency, but I find that hard to believe. It mostly occurs in the lower limbs, not the hands. His overall presentation is consistent with heart failure (or possible kidney issues).
Usually infusions in the hands are avoided in favor of ones in the arms, but given that this is the President, it's likely that they are preserving his arms for emergencies. Also hand infusions don't require him to take off his jacket and roll up his sleeves.

It's a long read, but Trump waxes poetic on topics such as the war in Iran, men in women's sports, and transgender mutilation - he even tells the kids not to listen to that part.
At this point many of us have gotten used to the way he sounds...but reading his words on the page is truly mind-blowing.
