It usually means they're no longer sexually attracted to the wife (and possibly already having an affair).

I think it happens all the time, but most men don't talk about it in the media before it happens

Mulaney's wife must feel awful about this sketchy timeline, but she really dodged a bullet.

I think he and his wife were likely separated before he went in to rehab. From her statement it sounds like maybe she thought they would get back together. I wouldn't be surprised if John was having an affair prior to rehab though.
