He's been telling you he's in pain for two weeks. I'd call that distress.

Food issues for my then 5 year old. Her belly used to hurt every day, and her legs used to hurt frequently. We noticed that when she stopped eating gluten free chicken nuggets for over a week(pure luck - the store ran out of them during the pandemic), the pain went away. She had a lot of tested allergies before then(wheat, eggs, dairy, some nuts). These nuggets seemed free of all the allergens, but they still gave her trouble.

It was really hard to distinguish the feeling from hunger because she could not describe what she was feeling. We took her to the pediatrician several times with no luck. The pain is completely gone now.

She is still very sensitive to different foods. She cannot have tomatoes, for example.

Try no dairy for a few days.

See if it helps.

So many kids are sensitive to milk and parents push them to consume more and more. Eventually they just feel sick all the time.

I have a DD who just turned 3 and she also complains about the tummy. She says it hurts, but it varies as to why. She says she ate too much, which Is what we say it would happen when she eats too much sugar. She also said that her tummy hurt because it was empty. (I think she heard my mom say that) But, she has been saying for the last two weeks that her tummy hurts off and on. She also doesn't show distress. A friend of mine who is a nurse told me that where she places her hand when she says it hurts, is not an area of concern and most likely is a call for attention or disruption in routine. I hate taking my kids to hospitals or Drs for no reason. I have too wasted time and money at the bequest of other people who freak out. I understand that is better to be cautious than not, but it sounds like the tummy pain is more common than what we think.

In my youngest two, it has been food allergies. But my oldest really was hungry!

My colleague's child was pointing to his side saying hurts at this age. Turned out to be a cancer... the child is in remission now after several rounds of treatments.

I don't want to freak you out, but it could be serious .Feel his tummy, make sure it's not hard, there's nothing weird going on there, monitor his eating, his behavior, etc. Jot it down for a few days. If it stays, I would just call a doc to check it and make sure all is well. Could be anxiety, could be gas, but if it was more serious, better to be ahead of it than not. Good luck.
