Could be depression. I've been there, and although mine was nowhere close to looking that bad, I was too depressed to even try to clean it regularly. Since I've been on medication it is clean again.

In the Post several years ago, I read the profile of a man who made his living cleaning cars. He said he would only do business outside of his neighborhood and circle, because the way a person maintains their car says a lot about them personally, and said something akin to what your therapist said. TMI, in short.
