By high school they were switching around clothes so much it was impossible for me to track. I was appalled at first that things weren't getting returned, but by the same token there were things of my DDs that had disappeared into other girls' wardrobes. So I figured it all evened out in the end and they had their own accountability system for making sure no one girl got out of hand with the "borrowing."
Happens for our kids too. They swap clothing and even winter coats but never undies though! we discourage the swapping but sometimes choose to pick our battles. If the other parent doesn't mind, then I wouldn't mind either. I would discuss what has come home when I see them so that they are aware.

Yes, I we can tell which clothing belongs to whom; I feel badly because DS has worn them more than once.
It's fine. It's not like it wore out. Just return it.

That's not an issue. Pull it out of his drawers, put it in a bag and give it to the mom. Although borrowing underwear from another kid is kind of gross. I'd give her a giftcard in addition for a new pack of underwear as a "thank you" for having lent the clothes.
Yes, just return them. (Washed and folded.) That your son wore them a few times is not a big deal. Do not include a gift card. That's just weird and stilted.

I get really ticked off when DD lends away clothing I bought her.
PP here. So was I at first. It came down to pick your battles and this one where I surrendered.
Ohh. Maybe it's because I'm poor, but I choose this battle.
Well she seemed to come out even in the clothes department with all this trading. The girls seem to have some intuitive accounting system that works in some kind of fair to teens way.
But as soon as I became aware of it, any new clothes were bought second hand or at the very most, and very occasionally, Target.
