Some airlines are really strict about this, and won't let you into the business class or first class areas unless you are the passenger named on the ticket.

+1. Also, we generally split things between our bags (I'll have extra batteries for the noise cancelling headphones, he'll have snacks, etc) so it's easier if we're next to each other.
I'm always able to select seats next to each other at booking for no additional fee. Not sure why others are claiming you have to pay extra. I've never had that problem.

You can choose "for free", but you have to pay a higher fare. What you want to to pay a bare-bones fare, and get premium perks.
