Is your relationship with her good enough that you could have a private conversation with her about the weight? Maybe give her some eating or weight loss tips? Sometimes, people know there's a problem, but don't know where to start in fixing it.

This is a very real side effect of many drugs for serious illnesses, so tread lightly, OP!
If you are NOT in fact a troll (or not a troll IRL), why don't you simply have a normal, kind, nonjudgmental discussion with no weight opinions whatsoever, particularly since many, many people overeat at Xmas and wouldn't appreciate overhearing your judgments?

I'm very confused. Do you want to warn his wife that if she doesn't lose weight, he'll leave her? Tell him that if he walks, she gets half? I cannot understand what wisdom you think you could possibly impart about the situation. Surely everyone is aware that she is now fat and he is unhappy, though those things aren't necesarily related.
