I've been wearing men's dress shirts for 25 years and have never had this happen. However, it is likely that the buttons need to be moved so that there is more room for the wrist. That way, when he bends his elbow, the cuff can travel down his arm, reducing pressure on the elbow. This is even more important on the left side, where he wears his watch.

We men are so mysterious.

I often get a hole in dress shirts where I put my right elbow on the center console when I drive.

I was just sitting here reading this thread at my computer and realized that I was using my right hand to mouse up and down, so my right forearm was resting on the desk, but I had my left elbow bent, with the point on the desk and with my hand on my chin. I suspect that, if you watch your husbands and the computer, they are doing the same thing.
