We got my mom a pay as you go plan through ATT. The phone was (I think) about 20 bucks and then you buy minutes through phone cards or can do automatic renewal. The more minutes up front you purchase the cheaper it is. I think it's 10 dollars for 100 minutes.

Virgin Mobile has good plans if you don't need/want unlimited and they start at $35/month. And, if you care about that thing, Virgin offers the iPhone 4 and 4s.

We have Virgin...their lowest rate plan is like $20/3months. We don't use phones much, just for emergencies, but rates are good and you buy in 3 month increments.
