Well, he says in the commercial, "it's a long way from West Africa," so clearly not the Middle East.

It must be that the actors got it on and then failed to keep it "off" when the cameras were rolling.

It's not a crotch grab though. The girl is sitting on her hand like a lot of kids do. The actor was trying to portray casual sitting.
