If this is not a troll it is a serious red flag. Two years? 90% vomiting if he goes alone!? She is doing something. Put your kid first and stop the visits.

Does she cook for him every time? Does this happen of they eat at a restaurant for dinner?

It sounds like severe anxiety. I would stop the visits, OP.

My BIL described a similar situation when he was in primary school, and was so stressed out due to many factors, including living in a country at war, that he would vomit the breakfast that his mother force-fed him every morning, when arriving at school. It's a core trauma memory for him.

Something about these visits is stressing out your son.

OP- I feel like your kid and grandma will be featured on Forensic Files in a few years. Something is happening. Supervised visits in your house or outings with you along and closely watching only

Does she have a cat? This is how I found out I was allergic to cats.

He is probably eating something at her house that she is not telling you about. My mom was notorious for feeding my son a whole bunch of sugar and lying about it. It was her way of buying his affection.

But when mom comes along he is fine. That’s extra odd.

This isn’t random or once off. This has been nearly every time he visits with her for two years! And he isn’t even in her house as you say- they often on outings. BUT- only when you aren’t with them. If you tag along, he is fine. She is definitely triggering something and I would stop these visits. Give it a pause for a year. Why would you subject your kid to this monthly? Something is going on.

You are saying you have tagged along as observer and he does not get sick?

My son has had episodes of random throw up and then better afterwards, but it has always been in the morning and has to do with acid build up during the night/long period of fasting. Not sure if that relates to your situation at all. Is he starving all day at school and then gorging himself or eating something that is particularly triggering? Does this happen when he is with her on weekends or something other than after school?

Could he go with her one day and you pack his own meal and then you can reassess after that? Just tell your mom that he’s been having some digestion issues and you packed snacks and dinner for him. Then you might be able to pinpoint it to see if it’s more food related or stress related.

Stress? Does she wear perfume/essential oils? Does she have animals that you dont have at home?
I get GI migraines but I dont vomit instead my migraine goes away after I have a #2 and its usually in combination with my period. I think the hormone drop must make me more susceptible. IDK. I do have them outside of that time but its because I am not watching what I am eating.

This happened to my DS 1/month for 6 months and finally we made the connection. It was always after eating a specific fast food. Later it would happen sometimes if they were dehydrated from sports, etc.
