Signs that designate parking only for Vets/Pregnant women are not legally enforceable. Signs that designate for pickup orders only are also not legally enforceable.
The trespassing notice is legally enforceable.

There were no close spots and I urgently need the medication and I’m in pain so I parked in the grocery pickup spot of which there were 4 and none of them were occupied. As I started walking into then store a manager stops me and tell me I need to move my car. I explain I’m sick and picking up drugs and will be in and out in 5 minutes. He block me from proceeding and tell me I have to move my car and it’s against store policy to do what I’m doing. I reply that there are 3 empty spots and I’m sick and just picking up meds. He tells me I can pick up my medication but I’m not welcome back. Who is in the wrong here?

Yeah, totally! Think of all of the entitled, lazy people who need to sit in their car in those spots while someone else shops for them.
Those people may be disabled and unable to do those things, or temporarily unwell (e.g. covid) and staying out of public space for public health reasons.

Once he asked, a couple of times, just move your car.
He was wrong to not just allow you to finish your quick business.
You were wrong in the things you don’t know / maybe he’s had complaints about this and the solution.. monitor these spots.
He was wrong in the things he doesn’t know. After my mom died, I wanted to .. like.. own all convenient spots and things for a while. I’ve also been so sick that I would park in a fire lane for goodness sake (and hope no fire or emergency).

I went straight from the doctor to the pharmacy so I could take the meds immediately.
