They actually seem to have done similar things with their cheeks and lips and it's good, subtle work that lets them have that plump, rosy cheek look and keeps their lips from totally disappearing (Flockhart has always had thin lips and they are thin now but not so thin as to be very aging).

B/c she's still thin.
I'm not here to sing the praises of being overweight, but I don't think being *that* thin typically helps with looking young.
It does when it's natural and just how your body is built. People who have to work really, really hard to maintain a size 0 or 2 age faster because their body is literally not getting enough nutrients. Middle age weight gain also hits them differently. In that case it's probably better to accept being a size 6 or 8 and focus on stength training and other maintenance.
But Flockhart is a naturally thin person with delicate bone structure. Like Audrey Hepburn. Audrey also aged well despite doing none of the Botox, fillers, and other interventions we do now (and that Flockhart is definitely doing to keep her skin looking smooth and youthful). But Hepburn wasn't dieting to keep her figure. She was just built like that and it aged well because it's what her body looked like. Same thing here, with the added benefit of modern beauty interventions.
Oh, also women with extremely feminine features age better because as we age, decreased estrogen can make our features look less feminine. But if you start out with very delicate, fine features, this effect is less pronounced even if you don't do HRT.

She still looks weird next to Grandpa Indiana Jones.
I agree. You can tell she's had work done, but it's subtle and in line with her natural aging. Honestly, she is an A++ specimen of how to do it correctly.

I bet she regrets marrying someone so much older.
It's Harrison freakin Ford! I doubt she regrets that!
