Oh I think they both know the score but I don't think you're right about the power balance in the relationship. First, they keep separate bedrooms and living quarters, and always had. Second, I think both are quite clear eyed about the unspoken contract in the marriage (and I do think that when they married he wasn't ugly or fat). And I think that Trump knows marital woes will not reflect well on him.
