+1 don't ever read the notes they are not only filled with inaccuracies I feel like most of these doctors straight up put themselves in a great light to avoid litigation then to put the actual events of a appt in them or to even speak the truth about the patient. They are so quick to paint you in a negative light then themselves.

OP I think you need to send in corrections then find a new doctor. Also make sure to review this doctor online and let other know why you left the practice.

You should definitely correct any inaccurate diagnoses from your problem list or medical history. But I wouldn’t spend a whole lot of time thinking about the “demeaning” language or whatever. The EMR systems that we all use now are so depersonalized that most of the language that you see is not free text, it’s generated from checking boxes and clicking here and there. It’s absolutely terrible. As a healthcare provider, I really lost so much of the narrative of the visit that has been a part of my career.

My PCP, OB, kid's pediatrician, are all part of the same medical group, and we have a patient portal where we can see upcoming and past appointments, pay bills, etc. Every visit has a post-visit summary with tons of info.

Correct the notes, send an email with both her notes and your correction. Tone should be respectful. Ask also who should you be sending the email to, if the doctor cannot correct her notes.

Speak out once, politely, firmly and in an email. I will guarantee that you will never have a problem again.

But the notes aren’t wrong. OP just dislikes how they express it.
