Do focus schools have an aide or other adult in the class with the kids?

Why would they put that in writing? The answer for max class size is up to the fire inspector. The schools couldn't care less how many students are crammed into a classroom. The more, the higher their salaries.

Not by default. It would depend on para staffing, and if there were students in the class needing extra support.

There is no cap. Unenforceable "guidelines."
But the PP is correct that CES classes are typically 28 students.
