Yikes. I have been with my NF for 7 years. I have 3 weeks PTO of my choosing, 5 days of holidays, and 5 days paid sick leave. Historically with this family I have used 3-5 of my sick days (I had a tumor removed one year and had a family member need heart surgery). Since COVID I actually haven’t taken any sick time since we are all pretty careful and I am saving my days in case I did get COVID.

I think your nanny is using way too much time and you need to talk to her. If she has a chronic health issue that requires her to take a lot of time off then you need to know, but if she is also taking a lot of legit vacations, I doubt that is the issue. Everyone I know with chronic health issues is pretty careful to save their PTO for sick days rather than taking a week off in the first quarter of a new job.

She is out for an unspecified medical issue. She had a doctors appointment today and the doctor told her to stay home till Thursday while they test. She was previously out with covid for five days which we were happy to pay for. I think we'll pay for her sick days last week and this week and then talk to her about it. I don't want to not pay her if she's really sick but we also need to find a long term solution.

I would replace her. She needs to at a minimum provide medical documentation and unpaid leave.
