Plus, my oldest brother and all of the oldest male cousins are named after their fathers. (Ie. my dad is "John Preston" and my brother is "John Donald."
We have to use both their first name and middle name, or we're never going to keep them all straight.
what about when there is only 1 mary, sarah, and margaret in a family? why then do they have to use both first and middle?

In the south the family name comes first. It isn't the middle name.
It's possible. That was what my friend did. Not sure if that's what everyone does.
