Just make sure you go to the official site. There are scam services that will charge a fee to do the application, which is totally unnecessary since it is a 5 minute process at most.

Why would you need a waiver? If you have a UK visa you don't need the ETA. If you are a US citizen you just go to the website (already posted in this thread) and do the application and pay the small fee. It is good for 2 years.
