The CC fees have gotten a lot higher, and a few years ago there was a SCOTUS ruling that allowed merchants to add a fee to cover it. Before that they were bound by the credit card agreements not to add their own fee.

The whole point of this thread is "now everywhere" - so many new places charging fees that never did before.
