Hard to tell. The growth spurts or maturing comes at different times and at different speeds. I had a classmate who was very childish and immature at 9. She told us that the longer she plays, the longer she stayed young. She found herself all grown or mature looking from 15- 17. She went from party to party and ended up having a child before 18. She had her second child at 30. Her 'maturing' or getting old seemed to have stopped and now at 42, she looks like 32. Nothing to do with her upbringing or family. Her 3 sisters didn't have these speeding ups and slowing downs.

Poor parenting.

From experience, I would say a combination of parenting style, birth order and innate tendencies.
I know the third category may be unusual for most families, but my middle sibling was the wild child of 5 children. She was lost in the shuffle, combined with dysfunctional parenting and natural tendencies to wander.
The older and younger children were pretty much straight laced. The dysfunctional family setup forced most of us to seek extracurricular activities/escape in sports and academics.
Long and short of it is - I don't know that there is a definitive answer, but there are certainly factors. I think even if we didn't have a dysfunctional set of parents, the middle child would have still been 'fast'.
