The hospital one I believe is a flashback to his youth when Rebecca was in the hospital.
I am the one that noticed his hair is much grayer as he walks to the door. His hair has barely any gray in some scenes from the cabin. I think one is Rebecca’s death and the other is Kate. Especially when Toby ask if she is ok with him being there. But he should be there to support his kids being there. Also, someone else noticed the walls to the door are different. This series is very specific with their details so I think it’s done on purpose.
I don’t think tonight’s episode will tell us who Kevin chooses based on his comments to Randall at the end of last week’s episode. I suspect we won’t know until the final episode.
I just rewatched this. I'm not sure why I am so obsessed with this mystery but here we are.
The door at Kevin's house is surrounded by a textured wall with some windows. You can't see the doorknob.
Then you see him opening a door surrounded by a flat tan wall. That doorknob is a horizontal silver handle. You never actually see him open that door.
Then there is the young Randall clearly in a hospital and you see young Randall open an industrial style hospital door.
But there is also this very short clip of adult Randall opening an industrial style hospital door and walking into a room -- it's blurry - but it looks like Rebecca in a bed, but isn't very sterile/hospital-ish.

Unless they're really screwed up the doors, I do think this is two deaths. We know the scene at Kevin's house takes place when his twins are about 12. Perhaps Rebecca is not in Kevin's house at all. Maybe everyone is gathering at Kevin's house to see someone else who is sick or dying. I thought maybe Kate, but we know she lives past this scene. Maybe it's Madison. When Randall says, "where is -" Beth says, "he's already in there." And we assume that's Nicky because he's at Rebecca's bedside. But maybe it's Elijah.
And the Rebecca death scene is taking place in a hospice somewhere, because we see the more industrial doorknob when Randall opens the door and walks in. And the only scenes we've seen of Rebecca's death are Randall walking into the room and saying hi mom, it's me Randall - your son and saying hello to Nicky. You can see windows behind Randall which make it seem like it's Kevin's house but they don't line up with the view we had before. However, it is not the industrial style doorknob behind him; it's a lever handle doorknob (but this doesn't line up with the lever dooorknob you see previously because there were no windows to the side of the door).

And when he asked "where is..." and the answer is "already in there"...remember that this scene is from when we didn't even know Rebecca was in the room, so they were holding back on even the fact that Nicky was there.
I'm all for a conspiracy theory but no matter how many times I watch it I can't see it another way.

I think they are trying to keep us guessing. It has turned into a mini Bachelorette.
