3. Kevin said he threw away all the model airplanes he and Jack made because he was so angry after his dad's death.
I've been sitting on this idea for awhile. I really hope I'm wrong but all foreshadowing seems to point to it.
I had the same thought when he gave Kevin his necklace at the end of the episode. It looked like Jack was saying goodbye to him. I forgot about those other points you made... I think this is the correct answer.

Maybe it was suicide but Rebecca never told the kids? She told them he died some other way so he wouldn't be "ruined" in their eyes. And Kate manages to make it be her fault.
