I had 2 kids at 2 different schools. Both times it was 4 total samples and parents could submit 2 to be considered for the packet.
My guess is your one AART is misinformed or miscommunicated to you.

More or less, yeah. My school sent the parent questionnaire in the same envelope as the in-pool letter, with instructions to please fill it out and return it. They didn't care about the 2 samples, but they wanted a completed questionnaire for each in pool kid.
