No, they don’t. I’ve never had one that has that info on it-and I now only ever carry the one insurance card, in my name, and it’s good enough for every dr we’ve ever seen for my two kids.

My 14 year old was vaccaros a county run clinic held in conjunction with the school district. No one asked for any proof of age.

While my 13 year old wasn’t asked for anything in Virginia, when we came out we were given a website to ask for my child’s vaccination record. It had EVERYTHING on it. - from the doctor, from the grocery store, even from the traveling nurse who was giving out flu shots at the Y. They do correlate, even if it doesn’t look like they are.

I’ve never had an insurance card with a birth date on it. But pharmacies will have your DOB.

DS took his drivers permit.

I was asked at an Arlington County site. I brought birth certificate, SSN card, insurance card, report card. By the second shot she had a learners permit.

Ask your pediatrician. Don’t “hope for the best” at a walk up clinic. Be proactive with the medics professionals you know.

They asked for an id from my 15 year old but I said he didn’t have one because I didn’t remember to bring it. All they did was ask him to state his DOB and that was apparently sufficient.

A few of DD's friends had issues with this because they were not born in the U.S. The birth certificate is in a different language for some of them.

OP here and thank you for the responses and advice. DD's birthday is next week and CVS does have her birthday on file as part of insurance so we'll try that first.

CVs didn't ask, but they also have the child's insurance card on file.
