One question in regards to the scratch- do you trim you baby's nails? They can get long and sharp and if there's a possibility that the baby scratched themselves it could explain why the caregiver didn't know how it happened.

Since there seems to be a pattern here, I think you are right to be concerned. But I also understand the feeling of not being sure what to do- my kids are at a center and I get calls for every minor scratch/fall for my rough-and-tumble preschooler (usually on the playground). Yet this same center failed to call me for something much more serious for my 2yo and I ended up in the urgent care with them on a Friday night recently. After talking with them I'm willing to give them a benefit of the doubt this time (2yo is not very verbal when wearing a mask) but I wouldn't be so understanding next time.

+1 Toddler, yes, but in the infant room DD only ever got an incident report for scratching her own eye. I'd be alarmed they don't know what's happening.

My baby has been at an in-home since she was 5 months old. She never had scratches, bumps or bruises before she could walk, not even in the crawling/cruising stage. Now at 2 she gets the occasional scrape and bruise. A couple of times she fell down hard while playing outside, but nothing serious. They always give us an incident report.
