Just get another medical field type job but just not in a hospital setting.
There are tons of them.

You get to make whatever choice you want to make, but I do think you should factor in risk and responsibility to your family right now.

My personal feeling is that this is too risky right now due to COVID. Your job sounds like it has some COVID risk. Then add that you will be pregnant, which lowers your immunity. Then add that you will be on your feet all day, which was rough during your last pregnancy. Then add that you are AMA for pregnancy, which adds some risk factors. Then add that if you're working, it means your 1-year-old is probably in daycare with other kids and more likely to get COVID there.

It seems like a bad idea to be working right now from you. That doesn't mean you're a SAHM forever -- it just means that you are not working due to your health and family needs. You don't have to commit to staying home for a few years for sure. You can take it slowly and see what makes sense after your second kid is born. Maybe there's another part-time job you could do from home or in a less-risky field that would be possible to do a few hours a week as well, keeping you doing something and earning some money but not pursuing your previous field.

I also think it will make sense on your resume to be able to say you needed to take some time off due to family risk and pregnancy during COVID. People understand this is an unusual time.

But again, this is what I would do. You also need to be comfortable and choose something you will be able to live with. If you're miserable, that's also not good for you and your family, even if what you're doing is the "logical choice."

I have also always worked and never wanted to be a SAHM, but if I were in your shoes right now, I would absolutely do it for the next year or so. My kids are school-aged, but I'm not sure I'd be sending them to daycare right now, especially if one was a baby, so if I could financially make it work, I would stay at home and just make all of our lives easier, even though it wasn't what I wanted to do long-term.

You say very little about your career, and what you do it seems like you don't like it that much , could take it or leave it.

It's a little tougher now, but you can socialiaze with other adults as a SAHM.

There's nothing wrong with changing your mind OP, ad admitting you do want to SAHM when you never thought you would.

It is also okay if after 6 months of being at home, your decide it's not for you at all and go back to work.

I do not know how taking 2 years off will impact your career, you will need to consult a radiation therapist board for that.

It doesn't have to be all or nothing

I read your post the same way, I get the impression you would actually prefer to stay home but need some validation. If I were in your shoes I would absolutely stay home for now.

Why complicate things?

I would quit for now and re-assess in a couple of years.

When it comes to your a job a lot might depend on how long you have been at. If you have been a med assistant since graduating college at 23 , and taking 2 years off may not be as disruptive compared to if you only started the career 1 or 2 years ago.

In either case it would be could to keep your license up to date, and with current trends, read journals or what have you, just so you aren't totally out of the loop when you go go on interviews when you want to get back into it.

OP - what is your current status at your work? If you didn't go back after the pandemic, are you on leave? Are they pressuring you to come back and you don't want to?

FWIW, it sounds like you want to quit, so I would quit. Plenty of people SAH for a few years and then go back to work.
